There seem to be three general types of clutch issues, or maybe two, I'm not sure:

1: Coffee grinder noise. It is my understanding that this is common, an if you bike does not make this noise, it will, unless you have the Kevlar clutch plates, or perhaps there are some brands of oil that help The cause of the noise is uneven wear on the clutch plates, aka scalloped plates. Clutch works fine, just makes noise.

2: Clutch slippage in 5th gear as experienced by KM. - not sure as to cause of this problem: - type of oil, weak clutch springs? Is it plate related?

For Slipping Clutch click HERE

3: Clutch slip/grab - this is dangerous and has caused at least one bike dump, as Knuckles has stated..Same problem as clutch Knuckles did replace his plates and problem is gone, and the places were not scalloped. Knuckles perhaps you can edify more on your symptoms, when they started occurring, what you did to compensate, etc...?
